Medical Forms and Screenings

Certificate of Dental Screening - Iowa Department of Public Health

All new elementary and middle school students as well as incoming Kindergarten students must submit this screening.

Certificate of Vision Screening - Iowa Department of Public Health

The vision screening shall be performed no earlier than one year prior to the date of enrollment in Kindergarten or Grade 3, and no later than six months after the date of the child’s enrollment in Kindergarten or Grade 3.

Athletic Pre-Participation Physical Exam Form

Every year, Grades 7 through 12 students shall present to the student’s superintendent a certificate signed by a licensed physician. This certificate is valid for one calendar year. A grace period, not to exceed 30 days, is allowed for expired certifications of physical examination.

Authorization to Give Consent For Treatment of a Minor

All students participating in athletics must submit a signed waiver. This form allows designated persons to give consent for the medical care of a specific child.
