Middle school students participating in athletics must sign and submit a Concussion Facts Sheet.
Athletic Pre-Participation Physical Exam Form
Every year, Grades 7 through 12 students shall present to the student’s superintendent a certificate signed by a licensed physician. This certificate is valid for one calendar year. A grace period, not to exceed 30 days, is allowed for expired certifications of physical examination.
Authorization to Give Consent For Treatment of a Minor
